Did you know that as a UNISON member if you have been off work for 6 weeks or more you get a £15 Love2Shop voucher? Fill in this form and your voucher will be sent out in the post!

 

Member Sickness Absence Notification

All UNISON members who have been off sick from work for 6 weeks or more are entitled to a Love 2 Shop Voucher from the branch. Fill in this form and the branch will post out your voucher to your home address
  • Please enter your membership number, if you cannot remember your membership number please email the branch at unison@southyorks.pnn.police.uk
  • If you are currently off work and have been so for 6 weeks or more you are entitled to claim a £15.00 Love2Shop Voucher. If you have been absent from work in the past 6 months and were off for 6 weeks or more you are still entitled to claim
